Mechanical Classification

Anatomical Levers

  • 1st Class (F in middle): Atlanto-occipital joint. Balance and stability; MA may be >1, =1, or <1.
  • 2nd Class (L in middle): Standing on toes (plantarflexion over MTP joints). Power; MA > 1.0 always.
  • 3rd Class (E in middle): Biceps brachii at elbow. Speed and ROM excursion; MA < 1.0 always. Most common lever in human anatomy.
Shoulder Kinematics

Scapulohumeral Rhythm

  • Overall Ratio: 2:1 overall (120° glenohumeral elevation to 60° scapulothoracic upward rotation).
  • Setting Phase (0°–30°): Primarily glenohumeral motion; scapula seeks stable base.
  • Force Couple: Upper/lower trapezius and serratus anterior drive upward rotation. Deltoid and rotator cuff (supraspinatus, subscapularis, infraspinatus, teres minor) compress and depress humeral head.
Locomotion Analysis

Gait Cycle Parameters

  • Distribution: Stance phase ≈ 60% of total cycle; Swing phase ≈ 40%.
  • Double Support: Occurs twice per cycle (~10% each, total ~20%). Disappears during running.
  • Initial Contact: Ankle dorsiflexed to 0° (tibialis anterior active eccentrically); knee near full extension (quadriceps active).
Interactive Practice in PhysioPilot: The mobile app contains 168 verified biomechanics questions with step-by-step distractor rationales, moment-arm calculators, and gait pathology quizzes.

Anatomy Practical Viva Checklist

High-yield nerve root values, muscle actions, and common examiner prompts across upper & lower extremity.

Muscle Name Primary Nerve Root Value Primary Clinical Action Common Viva Trap
Biceps Brachii Musculocutaneous nerve C5, C6 Powerful supinator at 90° elbow flexion; elbow flexor when supinated. Ineffective flexor when forearm is fully pronated (brachialis acts alone).
Serratus Anterior Long thoracic nerve C5, C6, C7 Scapular protraction and upward rotation; holds scapula to thoracic cage. Injury causes winged scapula on pushing against resistance (Bell's nerve).
Gluteus Medius Superior gluteal nerve L4, L5, S1 Pelvic stabilization during unilateral stance; hip abductor. Weakness causes contralateral pelvic drop (Positive Trendelenburg sign).
Iliopsoas Femoral nerve & L1–L3 roots L1, L2, L3 Primary hip flexor beyond 90°; increases lumbar lordosis if tight. Tested via Thomas test; tight iliopsoas keeps extended thigh off examination table.
Tibialis Anterior Deep peroneal (fibular) L4, L5 Ankle dorsiflexion and foot inversion; controls heel rocker eccentrically. Denervation causes foot drop and high-stepping (steppage) compensation.

Orthopaedic Special Tests Quick-Reference

Source-verified procedures, diagnostic clusters, and examiner pitfalls. Grounded in David J. Magee (6th Ed).

Lachman Test (ACL)

Gold Standard

Patient Setup: Supine, knee flexed to 20°–30°, hamstring completely relaxed.

Examiner Action: Stabilize distal femur with lateral hand; mobilize proximal tibia anteriorly with medial hand.

Positive Finding: Mushy or soft end-feel; anterior tibial translation >3mm greater than contralateral uninjured knee.

⚠️ OSCE Pitfall: Failing to relax hamstrings. Hamstring spasm holds tibia posterior and creates a false negative result.

Hawkins-Kennedy Impingement

Subacromial Cluster

Patient Setup: Seated upright, arm supported.

Examiner Action: Flex shoulder to 90° in sagittal plane with elbow at 90°; forcibly internally rotate the humerus.

Biomechanical Rationale: Greater tuberosity and supraspinatus tendon are driven beneath the coracoacromial arch and coracoid process.

⚠️ Diagnostic Note: High sensitivity (80%–92%) but moderate specificity (56%–67%). Must be interpreted within Park's Triad.

University PYQ Revision Strategy & Blueprint

Based on authentic West Bengal University of Health Sciences (WBUHS) and Indian health university examinations (2018–2024).

10-Mark Long Questions

Structured Scoring Blueprint

Examiners award marks systematically: 1) Definition & Etiology (2M), 2) Pathomechanics / Clinical Features (3M), 3) Detailed Assessment Steps (2M), 4) Phase-Wise Physiotherapy Management with exercise dosages (3M). Include a labeled diagram.

5-Mark Short Notes

Precise Tabular Answers

Do not write unstructured paragraphs. Always format comparisons (e.g. DeLorme vs Oxford, UMN vs LMN, Spasticity vs Rigidity) in clean 5-point comparative tables.

2-Mark Short Answers

Zero Fluff Definitions

State canonical definition verbatim, give the exact numerical threshold or normal range (e.g. normal Carrying Angle: 10°–15° females, 5° males), and state one clinical condition causing variation.

Resource Module 05 • OSCE & Bedside Assessment

Clinical Practical Examination Routine

Universal 7-step physical examination workflow tested by university external examiners.

1

Introduction & Informed Consent

Sanitize hands, introduce yourself with designation, verify patient identity, and explain what procedure will be performed.

2

Adequate Exposure with Draping

Expose both the affected and contralateral sound limbs for bilateral anatomical comparison while maintaining patient modesty.

3

Inspection Before Palpation

Observe posture, muscle wasting, swelling, surgical scars, and skin color before touching the patient. Touch the sound side first.

4

Active Range of Motion Before Passive

Assess patient's voluntary movement willingness and pain arc before applying passive overpressure or manual muscle testing.
